Canada Individual Visa Requirements

1. Duly accomplished draft application form - (You must answer every question. If not applicable, write N/A)
2. Duly accomplished form IMM 5476 (Use of Representative)
3. Duly accomplished Consent Form and Terms of Use for Residents for Services of VFS Global Services Philippines-Canada
4. Original valid passport (6 months valid) and old passport with valid US visa if applicable
5. Original personal bank certificate (must include account type, current balance, account opening date, 6 months average daily balance)
6. Original bank statement or certified true copy of passbook for the last 6 months
7. Any additional relevant documentation (last 2 pay slips, property titles, proof of pension, investments, etc.)
8. Copy of recent personal income tax return
9. Hotel booking confirmation (if applicable)
10. Roundtrip airline ticket reservation (if applicable)
11. Detailed itinerary during your trip (if applicable)

For Employees

• Original employment certificate (must include applicant's position, date hired, compensation, office address, landline numbers, and e-mail address)
• Copy of PRC / IBP ID (if professional)
• Copy of company ID

For Businessmen

• Copy of business registration from DTI or SEC
• Copy of business permit and mayor's permit
• Copy of PSA marriage contract (if traveling with spouse)

For Students

• Original school certification
• Copy of school ID
• Copy of PSA birth certificate (if traveling with immediate family)
• Copy of parent's documents, original bank certificate, original COE or business documents
• Affidavit of consent from the non-accompanying parent(s) or legal guardian (if traveling with only one parent or without parents)

For Retirees

• Copy of senior citizen card or retirement documents
• Copy of PSA marriage contract (if traveling with spouse)

For Foreign Nationals

• Copy of I-card

If the Company Will Shoulder the Trip

• Original employment certificate for sponsorship
• Copy of business registration from DTI or SEC
• Copy of business permit and mayor's permit
