Japan Individual Visa Requirements

1. Duly accomplished and signed application form (printed in A4 size paper)
2. One passport-sized colored picture (white background)
3. Original passport (6 months valid)
4. Original old passport with previous Japan Visa (if applicable)
5. Original bank certificate (issued within the last 3 months upon application) with 6 months Average Daily Balance or ADB. If the bank certificate has no ADB, a 6-month bank statement is required
6. Copy of latest income tax return (form 2316 with the signature of employee and employer; for business [TR], please attach a copy of receipt from BIR)
7. Original PSA authenticated birth certificate (issued within a year)
8. Original PSA marriage certificate (issued within a year – if married)
9. Two original authorization letters (for visa filing and pick-up)
10. Daily schedule in Japan

For Employees

• Original employment certificate (must include applicant's position, date hired, compensation, office address, landline numbers, and e-mail address)

For Businessmen

• Copy of business registration from DTI or SEC
• Copy of business permit or mayor's permit

For Students

• Copy of school ID
• Original letter of guarantee signed by the parent or guardian

For Retirees

• Copy of senior citizen card or retirement documents
• Original letter of guarantee if senior is dependent, signed by spouse, children, or sponsor

For Foreign Nationals

• Copy of i-card

If the Company Will Shoulder the Trip

• Letter of guarantee signed by the sponsor
• Copy of guarantor or sponsor's valid ID
• Original bank certificate of the company
• Copy of business documents and ITR of the company (If employed, company alphalist. If self-employed, receipt from accredited bank or BIR)

Special Notes

• If the trip will be shouldered by a family member, a proof of relationship and general requirements, including employment certificate/business documents of the guarantor is required.
• Additional requirements may be required by the embassy without prior notice.
• Personal appearance is not required (unless the Embassy requests it).
