United States Individual Visa Requirements
- Duly accomplished DS-160 online visa application form (must be submitted online)
- Appointment confirmation page (from the U.S. visa appointment website)
- Valid passport (must be valid for at least 6 months beyond your intended stay in the U.S. and must have at least one blank page)
- One recent 2x2 passport-sized photo (colored with white background; must follow U.S. visa photo guidelines and taken within the last 6 months)
- Visa fee payment confirmation (MRV receipt)
- Proof of financial capability to support your stay (original bank certificate and bank statements for the last 6 months)
- Proof of ties to the Philippines (employment, business, family, property, etc.) to demonstrate intent to return
- Travel itinerary or purpose of visit (optional but recommended)
- Previous U.S. visas and travel history (if applicable)
For Employees
- Original employment certificate with leave of absence (must include applicant's position, date hired, salary, and return to work date)
- Recent payslips
- Company ID
- Income Tax Return (Form 2316)
For Businessmen
- Business registration documents (DTI or SEC)
- Mayor's/business permit
- Income Tax Return (Form 1701)
- Financial statements
For Students
- School certificate of enrollment
- Copy of student ID
- Letter of leave of absence (if traveling during school period)
- Affidavit of support and consent letter signed by both parents
- Parent’s financial documents (bank certificate, ITR, etc.)
For Retirees
- Copy of senior citizen ID or official retirement documents
- Proof of pension or retirement income (bank statement, certificate, etc.)
For Foreign Nationals Residing in the Philippines
- Alien Certificate of Registration (ACR)
- Valid visa or residence permit in the Philippines
- Re-entry permit (if applicable)
If the Company Will Shoulder the Trip
- Company guarantee letter
- Company bank certificate and bank statements (latest 6 months)
- Business documents and ITR of the company
Special Notes
- The U.S. visa process includes an in-person interview at the U.S. Embassy or Consulate.
- Approval is not guaranteed and is subject to a consular officer’s discretion.
- Bring original documents during your interview, but submission is not required unless requested.
